Currently, I am at Metformin 850mg twice a day and my FBG is runing at around 100 +/- 5 points. I want to see if I can drop my Metformin to 500mg twice a day.

A: Your FBG can change within 24-72 hours, dpending on your metabolism.
Unless your doctor is authorising and monitoring this experiment, you are playing a dangerous game. No one should modify their medications without discussing it with a specialist.

A: This is called the Dawn Phenomenon, and pay attention to your body. When morning readings are higher than nightime, somethis is amiss somewhere.

LIVER DUMP can be fixed by a small snack at night (I have this) Do not eat sugary foods. Keep it healthy – a small amount of peanut butter, cheeze wheat toast, or my personal favorite – half a bar of Glucerna Snack Bar. Glucerna is made just for diabetics, unlike alot of other low-carb supplementals.
Also wake up a couple of times a week to be sure you aren’t dropping too low – too low for you would be best defined by your Endocrinologist.
MORNING INSULIN RESISTANCE – You need to discuss this with your doctor, and remember – if it is MIR then your carbohydrate choices for morning could cause high readings through midday or longer.

Q: my mother is diabetic type2.her blood glucose fasting levels are 205mg/dl this morning.?
After that she took daonil 5mg 1 tablet and then she had her breakfast in which she just have a cup of tea,a glass of milk,sugar free rusk and about half cup boiled black chickpeas.After break fast she took neodipar 850mg,then she checked her blood glucose 378mg/dl.
Difference b/w random and fasting is 173 which is very high.why her blood glucose is showing such a huge variation?

A: I don’t understand why , first of all that your mothers drugs are brand names. Neodipar is Metformin. I pay .01 cent for 500 mg. And daonil which is Glyburide is also very cheap. I just had to put that in.

Your mother should backhand her doctor and get another on NOW!!!!She surely does not have enough drugs. They need increased.

She also need to get on a low Glycemic Diet. Heres the website for 2,480 foods.

Q: Why do blood glucose levels fluctuate within minutes from a prior check?
Went to doc for physical this week and fasting blood glucose was 119; scheduled for recheck next week with additional follow-up if still high. This morning, having not eaten in 10 hours, I checked using my husband’s glucometer, and level was at 130. Checked the machine to ensure it was calibrated appropriately and checked again. Upon recheck, within minutes, it then read 117. (OK, still too high for fasting, but…..)
Why does it fluctuate like that?

A: No, that’s great. Fasting blood glucose should be between 70 and 100mg/dl. Your result is great.

That indicates that you are not at risk for hyperglycemia, or diabetes.

Hypoglycemia is defined as a glucose (or blood sugar) reading below 70mg/dl. This is likely what you would see on a meter if you were to test yourself when you feel lightheaded, dizzy, etc. in the afternoon, evening, and nights.

A: Normal Fasting Blood Sugar
A normal fasting blood sugar (which is also the blood sugar a normal person will see right before a meal) is:

83 mg/dl (4.6 mmol/L) or less.

Many normal people have fasting blood sugars in the mid and high 70 mg/dl (3.9 mmol/L) range.

Though most doctors will tell you any fasting blood sugar under 100 mg/dl (5.6 mmol/L) is “normal”, there are several studies that suggest that testing with a fasting blood sugar in the mid 90 mg/dl (5 mmol/L) range often predicts diabetes that is diagnosed a decade later.
Post-Meal Blood Sugar (Postprandial)
Independent of what they eat, the blood sugar of a truly normal person is:

Under 120 mg/dl (6.6 mmol/L) one or two hours after a meal.

Most normal people are under 100 mg/dl (5.5 mmol/L) two hours after eating.

Q: Why would fasting Blood Glucose be consistently higher than Bedtime levels?
I am trying to get my blood sugar levels into a normal range with exercise and diet. I am not on insulin or drugs and have not yet been diagnosed with diabetes but my MD is concerned about my fasting blood glucose.

What’s frustrating is that I am really watching what I eat and controlling my snacking and have a Bedtime BG of 95-104 (5.3-5.8) and yet my FBG seems to consistently be 108-110 (6.0-6.1). Why…where is the glucose coming from if I have been at rest and haven’t eaten for over 10 hours?

I thought it may be related to eating too late and/or too heavy a meal the night before and perhaps was still digesting after going to bed, but have watched and contolled what I have eaten during the last couple of days without any apparent impact.

A: When you fast, many times the liver thinks the bodies blood sugar is going too low, and it will release sugar into the blood stream. Diabetics call this ” dawn phenomenon”. It happens to many of us in the morning when we do our fasting blood sugar test, but a hour later, it is usually within normal range. It can also happen to diabetics when they exercise. It is just the liver trying to keep something from going wrong. You are right though, about your evening meal. If it was something with a lot of carbs, it can cause the fasting test in the morning to be high. Especially if you eat fiber like grains in bread, or eat oatmeal. If I have oatmeal, my blood sugar will stay up forever it seems. If I just eat a small amount of it, my blood sugar will rise some, but will stay stable for hours, I don’t need a snack. Next time you are going to have a test, make you last meal something lower in carbs and see what happens. Even non diabetics experience dawn phenomenon.

Q: My fasting blood glucose level is only 3.6mmol/L, is this healthy?
I tested it after water fasting for 12 hours because I have a family history of diabetes.

A: I would suggest that you do a range of tests over a week or so and see what the results come back as. When first diagnosed I was told to test twice a day and on the first day test fasting blood sugar on waking and before evening meal day 2 before lunchtime meal and before bed then fasting and evening meal with a few 2 hours after eating thrown in. A reading of anything under 4 is classed as a hypo for diabetics. Ideally your glucose levels should be in the range of 3.9 to 8.9 but if they are out of this range when you have tested for a week then see your doctor for more advice.

Q: fasting blood glucose readings – pregnant?
I am 9 weeks pregnant. I got tested early for gestational diabetes because my grandmothers had it. Anyway, in my 3 hour glucose test – everything was normal except for my fasting glucose (92) which they said was higher than normal. They said it has to be 90 or lower for pregnant women. So now, I am on a diabetic diet and regularly monitoring my blood glucose level for fasting and 1 hr after every meal.

My after meal readings are always well within normal range even if i happened to eat a little too much. As for my fasting glucose readings – one morning i took it and it said 95 (high) but then i figured, since my daughter was crying and screaming and was in a great big hurry to take it, i repeated it (since the dietitian said we should not be stressed when we do the glucose monitor)about 15 mins later and got a result of 88 (normal). I tried it again this morning and got a result of 94 the first time and 88 the second time from another finger. I know glucose monitors give different results and its acceptable as long as the numbers are not far from each other. But in my case, the few difference in numbers spells the difference between what is considered normal and what is considered high. So how should i bring this up to the doctor in my next appointment and is it really necessary to be going through all this just because i am a few points elevated? in fact, in other hospitals’ standards, my ranges would be considered all normal. Please give me some input. thanks :o )

A: Sweetheart … and I offer that as a general term of endearment. It is NOT intended to be offensive, obnoxious, condescending, or sexist … though, I must admit, I have yet to use it to an adult male, apart from my own two grown up sons and one of their friends who I’ve unofficially ‘adopted’ … I’m in agreement with your dietitian. I’m sad that your team of doctors and nurses didn’t explain things better to you as to why your blood sugar (glucose) level needs to be lower.

Sadly, raised blood sugar levels put your unborn child at additional risk compared to the trauma that s/he is already going through.

I’m quite convinced that the stress you seem to be experiencing from things not being properly explained to you will add to those risks.

As you say yourself, with “other hospitals’ standards”, your blood sugar “ranges would be considered all normal.” That’s true, in that a non-diabetics fasting blood sugar level would normally be between 70 and 99 mg/dL. It’s just that obstetric staff, in cahoots with their diabetes specialist colleagues know of the additional risks involved when a mother’s blood sugar levels are higher than ‘normal’.

I would suggest that you be open with your doctor(s) and ask them to explain just why your blood sugar level needs to be so tightly controlled. You can mention the fact that you’ve already stated about different hospitals’ standards with blood sugar level control, but I’m pretty sure you’re going to get a similar explanation as to that I’ve offered. Don’t let this put you off though. I firmly believe that knowledge puts you at the forefront of controlling the situation.
